Obtaining Services
  1. How can I move to an apartment or (group) home supported by Evant? 
    - By letting the County of Summit Board of Developmental Disabilities (CSBDD) Service and Support Administrator (SSA) know you wish to have Evant as a provider or
    -Notifying Evant’s Residential Director directly and asking about services.

  2. Can Evant provide services and supports in my own home?
    Services and supports can occur in many places: in a person’s own home or             apartment; in the family home; in a home owned by another person or even a home owned by Evant.  Availability and funding source can dictate where services are provided.

  3. How can I get a referral for Evant services?
    First a person must be eligible for CSBDD services.  The Intake Department at CSBDD can guide one through this process (telephone: 330-634-8686). Once found eligible for Board services, an SSA can be assigned.  The SSA can assist with a referral to Evant, or the Evant Residential Director can be contacted directly (330-920-1517) for referral information.
